To download the pdf donation form, click here. Then you can fill in the form and mail us your check. Our address is on the form, along with a phone number if you have questions.
Phone Contribution
If you prefer to make your donation by phone, please call us. We accept VISA and MasterCard. Mike Jones in the Finance Department will be happy to take the required information: (615) 743-3407.

- Bequests through a will or other estate plans
- Designated gifts to the United Way of Metro Nashville or of Williamson County

Beginning in the summer of 2008, the Senior Spirit will become Benchmarks, in keeping with the rebranding of our organization as FiftyForward.

With a strong commitment to honor the rights and privacy of our donors, FiftyForward does not disclose confidential donor information outside the organization. This policy has no exceptions. We do not sell or exchange a donor’s personal information with any other organization - public, private, or nonprofit.
