Here is a list of our programs and services, along with a brief description and contact information. You can also e-mail us at info@fiftyforward.org; your question or comment will be forwarded to the appropriate staff person in our organization. The area code for each phone number is 615.
Living at Home Services
Living at Home Services help seniors to remain living independently with supportive resources. These services are:
- Care Management – In-home assessment, information and referral; connection to programs and services; and advocacy are provided for senior adults. Madison /Northeast/Donelson 860-7180; Fairgrounds area 743-3416; other areas of Davidson County 743-3418; Williamson County 376-4334.
- Victory Over Crime – Seniors who have been victimized by any crime are supported through assessment, connection to services, advocacy, and in-home services. Call 743-3417. To volunteer for this program, call 743-3436.
- Contracted Services – Individuals who no longer are able to make safe and appropriate decisions are assisted through court appointment to receive care and manage finances. Services include private-pay care management and legal appointment as conservator of property or person. Call 743-3414.
- Respite Care/Senior Companions – Volunteers provide breaks for caregivers and visits for individuals who live alone. Volunteers are needed to provide this service, and eligible low-income seniors may receive financial assistance for volunteering. Call 743-3421.
- Senior Support Services – provide education and outreach regarding life adjustment issues and in-home professional counseling services. Call 743-3414.
- Meals on Wheels – Weekday and weekend hot lunches are delivered to seniors in designated areas of Nashville. Volunteer meal deliverers are needed each day; call 463-2264.
- Adult Day Services – Individuals with cognitive and physical limitations benefit from structured activities, social interaction, transportation, and nutrition. Group and individual volunteers are appreciated and needed for one-time and ongoing projects. Call 463-2266.
- In-Home Notary Services – Call743-3418.
FiftyForward Community Volunteer Opportunities
- RSVP – Volunteers' interests and skills are matched with community based programs. Call 743-3424.
- Friends Learning in Pairs (FLIP) – Senior adults are placed in public schools as mentors and tutors to elementary students, K-4, who are at-risk academically. Call 743-3422.
- Foster Grandparents – Low-income seniors receive financial assistance for providing one-on-one relationships and support for special-needs children. Call 743-3420.
Individuals of all ages: Adult Day Services, Meals on Wheels, Victory Over Crime, Respite Care, Senior Companions, Contract Services.
